Male Infertility

Dr. Craig S. Niederberger, MD

SEMEN ANALYSIS

Semen Analysis is important for any couple desiring fertility. Male factor could contribute to infertility in 30% of couples.

Male factor infertility is present in about half of couples seeking infertility investigation and testing. Semen analysis is a simple test that assess the sperm count, motility and normal forms and other parameters. It can help taylor the couple treatment to be more successful.  If you are trying to get pregnant and have a diagnosis that includes male factor infertility, consider scheduling a consultation with our experts to consider treatments.
Semen Analysis components:

  • Count - the number of sperm present in one sample
  • Motility - movement of the sperm
  • Morphology - percentage of sperm with normal shape

Indications for the semen analysis include:

  • Male factor infertility. One third of the couple has male issues contributing to the couple's infertility. We recommend that semen analysis be done in ALL men who desire to initiate a pregnancy. Semen analysis is a simple test that is very helpful to the treatment of infertility.
  • Prior to initiating ovulation induction in female parter
  • Recurrent pregnancy loss
  • Unexplained infertility
  • Hernia Surgery
  • Undescended Testicles

Dr. Samuel Ohlander, MD

MALE INFERTILITY: Intrauterine Insemination

Intrauterine insemination is  uses in men with abnormal semen analysis and or when times sex is not successful in achieving pregnancy.  When combined with ovulation induction, insemination increase the pregnancy rates two to four fold when compared to ovulation induction alone. Sperm are "washed" and then placed in the uterus during ovulation.

In most cases, IUI is done along with taking hormone medication to stimulate egg development. The medication can be oral (pills or vaginal suppositories) or injections. When a combination of oral and injectable medications are used, it is referred to as a hybrid cycle.Insemination increases the pregnancy rate ~ 2-4 folds when combined with ovulation induction.Indication for insemination include:

  • Male factor infertility. Many men has abnormal semen analysis. Low sperm count, decreased sperm motility or abnormal sperm shape can lead to infertility. This can often be treated with intrauterine insemination (IUI). It is estimated that 200 - 300 sperm are able to reach the egg during timed sex. Insemination allows millions of sperm to reach the egg thus increasing the chances for pregnancy.
  • Unexplained infertility
  • Endometriosis
  • Polycystic ovarian syndrome
  • When the couple fail ovulation induction with timed sex

Dr.Mahmoud Mima, MD

MALE INFERTILITY: In Vitro Fertilization  & ICSI

In Vitro Fertilization (IVF) with Intracytoplasmic Sperm Injection (ICSI) is a proven method that effectively overcomes barriers to achieving pregnancy for couples with low sperm count, low motility, and or low morphology.

Sperm Aspiration

Surgical Sperm Aspiration: We  provide outpatient surgical sperm recovery for men with low or no sperm, when it's a viable option. Sperm is collected, analyzed, frozen and stored so you only have to go through the recovery process once.

Vasectomy reversal service

We collaborate with urologist who offers clinic-based vasectomy reversals for men who qualify for the service. These procedures were previously only performed in an operating room. Success depends on the number of years since the vasectomy, age of the female partner and her reproductive history. A consultation is recommended to determine the best options. You can also call 832-330-6601 for more information.

Sperm Cryopreservation

Sperm cryopreservation is helpful to preserve the sperm before cancer chemotherapy and for  Veterans and other men traveling away from home.  Sperm cryopreservation after vasectomy reversal allows the long term storage of sperm at the time of surgery. Sperm are available in the event pregnancy is not achieved naturally after vasectomy reversal. You may then be eligible for a discount if you elect to do IVF with ICSI using your cryopreserved specimen.

If a male patient is seeking infertility care, it is important to have the female partner also meet with one of the fertility specialists.
